Available Form Templates in Auto Form Builder

Auto Form Builder includes six built-in templates. Each shows a description and field count on the template selection screen:

Blank Form

Description: Start from scratch with an empty canvas.

Fields: None — empty builder.

Best for: Custom forms with unique field combinations, multi-step layouts, or specialized intake flows.

Contact Form

Description: Name, email, phone, and message fields.

Fields (4): Full Name (required), Email Address (required), Phone Number (optional), Message (required textarea).

Best for: General contact pages, inquiry forms, and “Get in touch” sections.

Feedback Form

Description: Collect feedback with rating dropdown.

Fields (4): Your Name (required), Email Address (required), Rating dropdown (5–1 scale, required), Comments (optional textarea).

Best for: Customer satisfaction surveys, product reviews, and post-purchase feedback.

Support Request

Description: Support ticket with priority selection.

Fields (5): Full Name (required), Email Address (required), Subject (required), Priority dropdown (Low/Medium/High/Urgent, required), Description (required textarea).

Newsletter Signup

Description: Simple email subscription form.

Fields (2): First Name (required), Email Address (required).

How to Create a Form from a Template

Step 1: Go to Create New Form

In your WordPress admin, navigate to Auto Form Builder → All Forms and click Create New Form. The template selection screen opens automatically.

Step 2: Browse the Template Grid

The template modal displays all six templates as cards. Each card shows the template name, description, icon, and field count (or “Empty canvas” for Blank Form). New users also see a welcome message: Get started by choosing a template or create from scratch.

Step 3: Click Your Template

Click the template that matches your use case. Auto Form Builder creates a new form pre-loaded with that template’s fields and opens the drag-and-drop form builder.

Step 4: Customize the Form

The form title is set to the template name (e.g., “Contact Form”). Rename it, click any field to edit labels and settings, drag new fields from the panel, reorder fields, and configure Form Settings for notifications and success messages.

Save your changes, preview the form, and copy the shortcode

Customizing Templates After Creation

The plugin footer on the template screen confirms: You can customize any template after creation. Common customizations include:

  • Swap text fields for advanced fields — replace plain Full Name with the Name field for first/last name subfields
  • Add fields — file upload for attachments, address for shipping, checkbox for terms agreement
  • Remove fields — drop phone from Contact Form if you only need email
  • Change required status — make phone required or message optional
  • Configure notifications — enable admin email alerts and attendee confirmation emails in Form Settings
  • Style the form — use the form styler to match your site colors and typography
  • Add conditional logic — show or hide fields based on dropdown or radio selections

Template vs. Blank Form: When to Use Each

  • Template — your form matches a common pattern (contact, feedback, support, newsletter, event). Faster setup, fewer decisions.
  • Blank Form — your form has an unusual field mix, needs many custom field types from the start, or you prefer full control over every element.

Even blank forms benefit from the same drag-and-drop builder, Form Settings, and shortcode publishing — templates just give you a head start.

First-Time Setup Experience

When you install Auto Form Builder for the first time, a welcome modal appears with the template grid and the message Welcome to Auto Form Builder! Choose a template to create your first form and enter the builder immediately. Returning users see Create New Form with the same template selection when adding additional forms.
