Auto Form Builder includes 22 field types — all free.

Basic

Text, Textarea, Email, Phone, Number.

Choice

Checkbox, Radio, Dropdown (standard, searchable or multi-select).

Date & time

Date, Time, Month & Year.

Advanced

Name, Address, File Upload, Signature, Color Picker, Calculation (live totals from other fields), Products (order items, quantities, and a live total), and Page Break (for multi-step).

Survey

Star Rating, NPS, Opinion Scale, and Tabular Survey (Likert grid).
